Schedule Details: This 1.0 FTE commits to 40 hours per week. Four 10 hour shifts, shift starts between 6am to 10:30am. Rotate between 3 hospitals - MUK, OMH, PWK. Every 3rd weekend rotation.
About Us:
The ProHealth Retail Pharmacy Department has four locations located throughout Waukesha County, including Oconomowoc Memorial Hospital, Pewaukee Cancer Center, Mukwonago Campus and Waukesha Memorial Hospital. Our four pharmacies fill approximately 5,000 prescriptions monthly while focusing on a personalized, one-on-one interaction with each patient. The ProHealth Pharmacy staff collaborate with other healthcare providers to ensure all patients’ prescription needs are taken care of before going home. We primarily fill prescriptions for employees, patients discharging from the hospital, hospice patients, oncology patients, or patients seen by the Medical Group. The pharmacies’ integration with the rest of the healthcare system helps to improve our patient experience and allows our staff to provide more informed, safe care.
What You Will Do:
Assists pharmacists, physicians and nurses with medication reconciliation processes to ensure that any differences between medications taken before admission and medications ordered upon admission for a patient are intentional and void of negative patient incident. Assists with reconciliation process at discharge in coordination with pharmacist and case manager. Offers discharge prescription service through ProHealth Retail Pharmacy. Collects appropriate insurance and demographic data for prescription processing.
Requirements:
- High School Diploma or equivalent
- Pharmacy Technician (CPhT) within 2 years of hire and registered with State of Wisconsin
- Valid WI driver's license and in good standing
- Basic Life Support Certification (BLS)
