Responsible for stroke program throughout PHC

Job Summary

Job Summary Coordinates the center of excellence for stroke care at ProHealth Care and provide professional, clinical nursing, technical knowledge, skills, and expertise required for the provision and continuity of comprehensive neurological and stroke patient care.
This individual will also be responsible for ensuring that the clinical, programmatic, and quality outcomes are met/exceeded and ensure that patient needs are met throughout the continuum of care.
ProHealth Care currently has two hospitals certified as Joint Commission Primary Stroke Centers located at Waukesha Memorial Hospital and Oconomowoc Memorial Hospital.


• Collaborates with our stroke medical directors and stroke team in program coordination and in development/enhancements to the stroke protocols and ordersets as needed.
• Develop plans to incorporate research into practice.
• Continuous monitoring for practice adherence.
• Understands and implements the regulatory standards we are held to by the Joint Commission and Get With the Guidelines.
• Oversight and organization of Joint Commission site visits.
• Analyze data and report data to appropriate committees.
• Implements process changes to continuously improve the care of our patients.
• Oversees and facilitates the Waukesha County Stroke support group,
• Coordinate educational events with EMS in Waukesha County.
• Assists in implementation of strategic plan regarding stroke patients and collaborates in the budget process for the stroke program.
• Specifically addressing barriers for patient compliance with medications, address psychosocial wellbeing, environmental issues and assess patient's functional status on an outpatient basis.
• Participate in inpatient rounding at both hospitals and assist the nursing staff with educating our stroke/TIA patients.
• Will be required to assist with educational presentations to other healthcare professionals and the community.
• Will be required to manage the community stroke support group including coordinating speakers, organizing meetings and communication with group members.
• Additional responsibilities including data abstractions and coordinating stroke meetings.
• Collaborate with abstractors for data submission.
• Oversight of inpatient rounding and concurrent compliance review.
• Developing and coordinating education for staff to include nursing staff and medical providers.
• Develop competencies for staff to include annual CBLS and NIHSS recertification.
• Coordinate Stroke Boards case reviews and meetings.
• Coordinate and lead stroke committee.
• Communicate organizational and group updates efficiently.
• Stroke Coordinators of Wisconsin (SCOW), Stroke Coalition, and Coverdell membership and related activities.
• Attend educational seminars and conferences; incorporate information into practice.
Participate in research related activities related to the care of stroke patients.
