Schedule Details: Sat and Sunday days 9-1730. On site role at either WMH or PHHC Home Riverwood office Must be able to train at minimum 2-3 days during the week.
Starting at $19.87 (increases with experience)

Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
Job Summary
What You Will Do:
Coordinates the intake of new patients to Home Health. Seeks out needed information from appropriate resources to complete the intake process. Collaborates with referral source, physician and patient/family to plan for future agency services.
What you will need:
- High School Diploma or equivalent required.
- Home Health/Hospice experience preferred.
- Customer service experience interacting with patients strongly preferred.
- Referral experience preferred. *CA #LI-SS
