ProHealth Care offers quality in-home hospice care and in-home skilled nursing care, as well as a 15-bed residential, free standing, inpatient hospice. Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
What You Will Do:
The Community Liaison for ProHealth Home Care and Hospice is responsible for promoting awareness and availability of home care and hospice services to a multitude of referral sources in the community. This role is responsible for acquiring and maintaining industry expertise and education regarding ProHealth Care’s home care, palliative, and hospice services. Acting as a liaison between the customer and our agency, this person will develop and expand relationships to increase utilization of home care and hospice services.
Responsibilities:
- Demonstrates initiative and commitment to achieving company growth and results to ensure long-term viability.
- Represents ProHealth Care homecare, palliative, and hospice services positively and professionally within the community.
- Provides accurate information regarding homecare, palliative, and hospice services in response to inquiries by healthcare providers and general public, in accordance to the goal-directed development plan.
- Maintains current data on market area, competitors, and marketing strategies.
- Maintains an organized approach to territory management.
- Prepares and conducts call and presentations to potential referral sources.
- Participates in strategic planning and the analysis for their assigned territory in conjunction with the business plan.
- Coordinates with clinical management and business development in planning in-service and presentations, and in addressing issues with referral sources.
- Participates in community and organizational programs as requested to promote professional growth and understanding of homecare, palliative, and hospice services.
- Primary contact for intake questions regarding patients in facilities
Requirements:
- High School Diploma or equivalent; bachelors degree preferred.
- Medical background preferred
- Minimum of 3 years home care and hospice experience
- Valid driver’s license and vehicle for local travel
- Excellent customer service skills with experience interacting with patients strongly preferred *CA #LI-MO
