Clinical Social Worker – Complex Care Management
ProHealth Care | Full Time | Day Shift
At ProHealth Care, our Complex Care Management team supports patients with high levels of medical, cognitive, and social complexity. We are currently seeking an experienced Clinical Social Worker to join this specialized team and partner closely with RN Complex Care Managers to support patients and families navigating significant challenges.
This is a unique social work role designed for clinicians who are motivated by complexity, problem-solving, and patient advocacy. Many of the patients supported through CCM experience cognitive impairment, dementia, and behavioral health needs.
What You’ll Do:
• Partner with RN Complex Care Managers to support patients with complex needs
• Conduct psychosocial assessments and identify barriers impacting care and safety
• Support patients with cognitive challenges, dementia, and behavioral health needs
• Assist with Medicaid applications and connection to community resources
• Complete home visits to assess patient needs and living environments
• Coordinate care with community agencies and healthcare partners
• Advocate for patients and families while meeting them where they are

Qualifications:
• Bachelor’s degree in Social Work Required, Masters preferred
• Wisconsin social work licensure required
• 3–5 years of social work experience required
• Experience in case management, community-based services, behavioral health, and/or dementia care preferred

What You Will Do:
Responsible for the timely effective and efficient identification and counseling of patient(s) and their significant other(s) who require psycho-social, emotional and financial counseling in his or her assigned medical center area(s). This worker performs a variety of social work functions in accordance with the accepted principals and standards of the Social Work Practice.

- Bachelor's Degree in Social Work
- Licensed Social Worker (WI)
