Schedule Details: This is a pool/ as needed position, hours of shifts are typically 0800 to 1430 (8am- 2:30) day shift or 1400-2230 (2pm-10pm)/ 1000 to 1830 (10am- 6:30pm) PM shift. Must commit to picking up one shift per week/ 4 shifts per month. One weekend per 6 week schedule, one holiday per year. This is not a seasonal or temporary position.

Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.

What You Will Do:

The Triage and Visit Nurse (internally titled RN - Home Hospice) in Home Care and Home Hospice works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment.  

 

This nurse may develop the plan of care, follow an established plan of care, or make modifications based on their assessment of the patient. This nurse for home care & home hospice is responsible for improving the health of patients dwelling in their own homes. This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. They provide hands on care in the home environment and are offered autonomy, flexibility and the luxury of caring for one patient at a time. 

 

What you will need:

  • Associate’s Degree in Nursing required. Bachelor's Degree in Nursing preferred.
  • Previous Hospice Nurse/ Home Care experience 
  • Current State of WI or eNLC compact state RN license.
  • Valid State of WI driver’s license with an acceptable driving record.
  • Current BLS (CPR) certification or ability to obtain upon hire.
  • Clinical experience in home health preferred.
  • EPIC software experience preferred.
