Schedule Details: Will rotate between 4 work locations - Mukwonago, Oconomowoc Clinic, Heart & Vascular and Pewaukee. Hours will be predominantly M-F 8-4:30
What You Will Do:
The clinical pharmacy coordinator is responsible for coordinating clinical pharmacy services ensuring that the pharmacy provides optimal clinical services on a daily basis. The clinical pharmacy coordinator facilitates the development, implementation and maintenance of systems that promote desirable patient outcomes through the use of safe cost-effective drug therapies. This position reports to the Director of Pharmacy and provides pharmaceutical care to the types and ages of patients defined by the “Populations Served” above.
Requirements:
- Bachelor's degree in Pharmacy
- Pharmacist (R.Ph. or RPh)
- Basic Life Support Certification (BLS) required
- ACLS and PALS certification preferred
- WI Driver's License
- Hold and maintain BCPS (Board) certification in pharmacotherapy.
- 4 years of relevant experience preferred
