Nurse Practitioner or Physician Assistant, Fully Paneled - Delafield Primary Care - 1.0 FTE

We are seeking a highly skilled and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our Delafield Clinic as a fully-paneled provider, with your own patient panel and full practice authority. In this role, you will be a critical member of our primary care team, delivering comprehensive care to patients across the lifespan. This position is ideal for an advanced practice provider with a strong foundation in family medicine or primary care who is ready to manage a full panel of patients independently.

The Delafield clinic is one of PHMG’s 15 clinic locations in Waukesha County, offering a personalized care experience for both adult and pediatric patients. With a focus on Internal Medicine and Pediatrics, our clinic is known for its patient-centered approach, fostering long-term relationships with families and individuals. What You Will Do:

 

  • Provide high-quality, compassionate care to patients of all ages.
  • Independently manage a panel of patients, including assessment, diagnosis, treatment, and follow-up care.
  • Utilize your clinical expertise to make sound medical decisions and provide patient education.
  • Collaborate with a multidisciplinary team to ensure comprehensive care.
  • Maintain accurate and timely medical documentation.

What You Will Need:

Licensure:

  • Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating NLC state (for NPs).
  • Certification as an Advanced Practice Nurse Prescriber by the Wisconsin State Board of Nursing (for NPs).
  • Physician Assistant license issued by the state of practice (for PAs).
  • DEA registration or eligibility to obtain.

Certification:

  • ANCC or AANP certification for Nurse Practitioners.
  • PA-C certification by the NCCPA.
  • AHA Basic Life Support for the Healthcare Provider (BLS) or an equivalent certification.

Education:

  • Master’s degree in Nursing for Nurse Practitioners.
  • Bachelor’s degree for Physician Assistants.

Experience:

A minimum of 5 years of clinical experience in family medicine or primary care.
