What You Will Do:

  • Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA) with licensure in the state of Wisconsin.
  • 5+ years of experience in family medicine or primary care.
  • Ability to work independently and manage patient load. 
  • Strong clinical knowledge and competence in assessment, diagnosis, and treatment.
  • Commitment to providing compassionate, high-quality care to patients across the lifespan.

 

Licensure:

Current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating NLC state.

Certification as an Advanced Practice Nurse Prescriber by the Wisconsin State Board of Nursing.

Certification by ANCC or AANP for Nurse Practitioners or Physician Assistant license issued by the state of practice.

PA-C certification by the NCCPA.

DEA registration or eligibility to obtain.

 

Certifications:

AHA Basic Life Support for the Healthcare Provider (BLS) or an equivalent certification.

 

Education:

Master’s degree in Nursing for Nurse Practitioners.

Bachelor’s degree for Physician Assistants.

 

Experience:

A minimum of 5 years of clinical experience is required for this fully paneled provider.
