Our Home Health & Hospice Weekend Program combines 12-hour shifts, premium weekend pay and meaningful one-on-one nursing care with the independence of working in the field. You’ll care for patients throughout Waukesha County and portions of surrounding counties while having the support of an experienced interdisciplinary team behind you.
This is a great fit for an RN who values autonomy, strong patient relationships and a schedule that looks different from the traditional five-day workweek.

Friday, Saturday & Sunday | 12-hour shifts
Rotating hours: 8:00 a.m.–8:00 p.m., 9:00 a.m.–9:00 p.m. or 10:00 a.m.–10:00 p.m.
Work three weekends on, then enjoy one weekend completely off, that’s 12 weekends OFF each year.

Home Health nursing gives you something that can be hard to find in a traditional inpatient setting: the opportunity to focus on one patient at a time.
You’ll get to know your patients and families, see how they’re actually managing at home and use your nursing judgment to make a meaningful difference in their care. You’ll work independently in the field, but you won’t be on your own, you’ll collaborate with nurses, providers and the broader Home Health & Hospice team throughout the day.
What you’ll do
Provide skilled nursing care to Home Health and Hospice patients in their homes, including assessments, medication management and education, wound care, IV therapy and other skilled nursing needs. You’ll identify and communicate changes in condition, educate patients and caregivers, coordinate with the interdisciplinary team and document care in the electronic medical record.
What you’ll need
- Current Wisconsin RN license or eligibility to practice in Wisconsin, BLS, a valid driver’s license and the ability to travel throughout the assigned service area.
- Home Health or Hospice experience is valuable, but strong nursing experience, independent clinical judgment, organization and the ability to manage changing priorities are equally important.

What You Will Do:
As a Visit RN, you’ll work one-on-one with patients in their homes to provide skilled nursing care, coordinate services, and contribute to the patient’s overall care plan. You’ll collaborate with a dedicated team of nurses, social workers, physicians, caregivers, and community agencies to ensure a holistic, coordinated, and compassionate care experience.
- Deliver individualized, evidence-based nursing care in the home setting
- Assess patient status and communicate any changes in condition to the care team
- Document care in EPIC, our integrated electronic health record system
- Collaborate on discharge planning, care coordination, and resource utilization
- Serve as a clinical liaison with physicians and other members of the care team
- Educate patients and families on self-care, medications, and condition management
What You’ll Need:
- Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred
- Current WI RN license or eligibility through the eNLC compact
- Valid WI driver’s license and acceptable driving record
- BLS certification (or ability to obtain upon hire)
- Previous experience in home health or hospice is preferred
- Familiarity with EPIC and experience in case management or discharge planning a plus
- Strong interpersonal, communication, and critical thinking skills
