Looking for an RN role that offers more independence, one-on-one patient care and the ability to manage your day outside of a traditional hospital setting?

Join ProHealth Home Care & Hospice as a Visit RN and provide skilled nursing care to Home Health and Hospice patients throughout our community. You'll have the autonomy of field nursing with the support of an experienced interdisciplinary ProHealth Care team behind you.

 

Schedule & Territory

  • 0.6 FTE – 24 hours per week 

  • Three days per week, 8:00 a.m. – 4:30 p.m.

  • Every third weekend

  • Holiday rotation – two of six holidays

  • Coverage includes both Home Health and Hospice patients.

  • Territory: Waukesha County and portions of surrounding counties.

  • Mileage reimbursement is provided at the current federal rate for eligible travel between patient visits.

What your day looks like

You'll receive your anticipated patient workload ahead of the workday, connect with the team during the morning huddle and coordinate visit times with your patients.

From there, you'll manage your visits throughout the day, traveling within your assigned service area and adjusting as patient needs change. Rather than caring for several patients simultaneously, home-based nursing allows you to focus on the patient and family in front of you.

Your visits may include:

  • Nursing assessments and monitoring for changes in condition

  • Wound and ostomy care

  • Medication education and management

  • Injections and IV therapy

  • Patient and caregiver education

  • Chronic and complex disease management

  • Comfort and symptom management for Hospice patients

  • Communication with physicians and other members of the care team

  • Coordination with nurses, therapists, social workers and other disciplines

  • Documentation in Epic

 

What you'll need

  • Current Wisconsin RN license

  • Current BLS certification

  • Valid driver's license and acceptable driving record

  • Strong assessment, critical-thinking and communication skills

  • Ability to independently prioritize patient needs and manage a field-based schedule

  • Excellent time management skills

  • Approximately two years of RN experience preferred

  • Home Health experience preferred

 

Experience in acute care, medical-surgical nursing, wound or ostomy care, IV therapy, infusion, oncology, rehabilitation or other hands-on clinical settings may translate well to this role.
