RN Case Manager – Home Health | 1.0 FTE Days

At ProHealth Home Care, our patients are right here in Waukesha County and nearby communities. You won’t be covering half the state. Our RNs serve local territories including Waukesha, Pewaukee, Brookfield, Elm Grove, Menomonee Falls, Delafield, Hartland, Mukwonago, Big Bend, Oconomowoc, Dousman, Eagle and surrounding communities.

As an RN Case Manager, you’ll have the independence to organize your day around your patients, use your clinical judgment and manage their care from admission through discharge. You’ll get to know your patients in their own environment, understand what they really need to be successful at home and have the time to educate, problem-solve and build relationships.

 

You’ll work independently, but you won’t work alone. Our RNs are supported by an experienced interdisciplinary team of physicians, therapists, social workers and other Home Health professionals.

 

And because you're salaried rather than paid per visit, your compensation remains consistent while your clinical decisions stay focused on what your patients need.

 

A schedule with flexibility built in

 

  • This is a 1.0 FTE day position, primarily Monday through Friday, 8:00 a.m. to 4:30 p.m.
  • You’ll have flexibility to organize your daily visits around patient needs while meeting established productivity expectations. The position includes approximately one to two backup on-call nights per month and a rotating holiday commitment.
  • A sign-on bonus is also available for eligible external candidates with a two-year commitment.

Your territory is local and manageable, with typical daily driving of approximately 50–70 miles. Eligible work-related mileage is reimbursed at current federal rates.

  • Cell phone

  • Mobile Wi-Fi/hotspot

  • Epic for clinical documentation

 

What your nursing looks like

 

You’ll own and coordinate the plan of care for your patients, including assessments and OASIS, medication management and education, wound care, injections, IV therapy, symptom management, patient and caregiver education, and coordination with physicians and the interdisciplinary team.

You’ll help patients transition safely home from hospitals and rehabilitation facilities, monitor their progress and adjust the plan as their needs change.

Most importantly, you’ll have the opportunity to see the whole patient, not just the reason they came through the door.

 

What you’ll bring

 

  • Associate degree in Nursing required; bachelor’s degree preferred.
  • Current Wisconsin RN license or qualifying Enhanced Nurse Licensure Compact license.

  • Current BLS certification.

  • Valid driver’s license, acceptable driving record, vehicle insurance and reliable transportation.

  • Strong clinical assessment, critical-thinking, organization and patient-education skills.

  • The confidence to work independently and the judgment to know when to lean on your team.

  • Comfort providing care in a variety of home environments, including homes with pets, smoke or allergens.

 

Previous Home Health, Hospice, case management, acute care or post-acute experience is valuable, but above all, we’re looking for an experienced RN who enjoys independence, relationship-based care and helping patients succeed at home.
