Join ProHealth Home Care & Hospice as a Pool Visit RN. You'll provide skilled nursing care to Home Health and Hospice patients in their homes while having the flexibility of an as-needed schedule and the support of an experienced interdisciplinary ProHealth Care team.

 

This can be a great opportunity for an experienced RN looking to supplement another position, maintain clinical practice with a flexible commitment or explore Home Health & Hospice nursing.

 

Schedule & Territory

  • Pool/PRN – as needed

  • Minimum availability of two shifts per month

  • Flexible 4-, 8- and 12-hour shift opportunities, Day and PM coverage Options, Weekday and weekend opportunities

  • Weekends OR on call -  every third weekend, 8-hour shifts Saturday and Sunday, or an on-call rotation approximately two to three times per month

  • Coverage includes both Home Health and Hospice patients.

  • Territory includes Waukesha County and portions of surrounding counties.

  • Visit RNs may drive up to approximately 75 miles during a workday. Eligible business mileage is reimbursed at the current federal mileage rate.

What your day looks like

As a Visit RN, you'll travel to patients' homes within the service area and provide skilled nursing based on each patient's individual needs and plan of care.

You'll coordinate your visits with patients and have autonomy in organizing your day while responding to changing patient needs.

 

Your visits may include:

  • Nursing assessments and monitoring for changes in condition

  • Wound and ostomy care

  • Medication education and management

  • Injections and IV therapy

  • Patient and caregiver education

  • Chronic and complex disease management

  • Comfort and symptom management for Hospice patients

  • Communication with physicians and other members of the care team

  • Coordination with nurses, therapists, social workers and other disciplines

  • Documentation in Epic

 

What you'll need

  • Current Wisconsin Registered Nurse license

  • BLS certification or ability to obtain

  • Valid driver's license and acceptable driving record

  • Strong nursing assessment, critical-thinking and communication skills

  • Ability to independently prioritize patient needs and manage a mobile, field-based workday

  • Comfort traveling between patient homes throughout the service territory

  • Comfort providing care in a variety of home environments, which may include pets, tobacco smoke and other common household allergens or environmental conditions

  • Previous clinical nursing experience preferred

  • Home Health, Hospice, case management or discharge-planning experience preferred

 

New to Home Health & Hospice?

Strong nursing experience from other settings can translate well to home-based care. RNs with experience in acute care, medical-surgical nursing, wound or ostomy care, IV therapy, infusion, oncology, rehabilitation and other hands-on clinical settings may already have many of the assessment, education and critical-thinking skills needed for this work.

For nurses new to Home Health & Hospice, orientation includes dedicated training to help you learn the workflows and expectations of providing care in the home.
