Visit RN – Home Health & Hospice - Full Time - 1.0 FTE Days

As a Visit RN with ProHealth Home Care & Home Hospice, you’ll provide one-on-one skilled nursing care to both Home Care and Hospice patients throughout Waukesha County and portions of surrounding counties. You’ll have the independence to manage your visits and use your clinical judgment in the field, with an experienced interdisciplinary team behind you.

Your schedule & territory
1.0 FTE | 40 hours per week
Monday–Friday | 8:00 a.m.–4:30 p.m.
Every third weekend | Holiday rotation
You’ll care for both Home Care and Hospice patients throughout Waukesha County and portions of surrounding counties. Your workday is spent traveling between patients’ homes, with eligible work-related mileage reimbursed. ProHealth also provides your laptop, cell phone and mobile Wi-Fi/hotspot so you have the technology you need in the field.
What you’ll do
Home Health nursing gives you the opportunity to see the whole patient not just their diagnosis. You’ll see how they’re managing at home, build relationships with patients and caregivers, and use your nursing judgment to help them remain safely and comfortably in the place they want to be.
You’ll provide skilled nursing care including assessments, medication management and education, wound care, IV therapy and other skilled nursing needs. You’ll identify and communicate changes in condition, educate patients and caregivers, coordinate with providers and the interdisciplinary team, and document care in the electronic medical record.
You’ll work independently in the field, but you won’t work alone. Your Home Health & Hospice team is there for collaboration, clinical support and coordination when you need it.
What you’ll need
Current Wisconsin RN license or eligibility to practice in Wisconsin, current BLS or ability to obtain upon hire, a valid driver’s license and the ability to travel throughout the assigned service area.
Home Health, Hospice or case management experience is valuable, but strong transferable nursing experience matters too. We’re looking for nurses who are organized, confident working independently, comfortable managing changing priorities and passionate about helping patients and families navigate care at home.
