Schedule & Location
This position is 0.6 FTE, 24 hours per week, working three 8-hour days, typically 8:00 a.m.–4:30 p.m. The schedule includes every third weekend and a rotation of 2 of 6 holidays.
The team connects during a 7:45 a.m. huddle to confirm the day's patient needs. Visit RNs then coordinate directly with their patients to organize their visits throughout the day.
The department is based at ProHealth's Riverwood Administration Building in Waukesha, but this is a field-based role. Visit RNs travel throughout ProHealth Home Care & Home Hospice's service area based on patient needs.
What You'll Do
Provide skilled nursing care to Home Health and Hospice patients in their homes
Perform assessments and monitor changes in patient condition
Provide wound and ostomy care, IV therapy and infusion care
Support symptom and pain management for Hospice patients
Educate and support patients and caregivers
Collaborate with RN Case Managers, physicians and the interdisciplinary care team
Document patient care in Epic
Visit RN productivity is based on weighted units that account for the type and complexity of care provided, not simply the number of patients seen per day.
What You'll Need
Current Wisconsin RN license or eligible Nurse Licensure Compact license
Valid driver's license in good standing and reliable transportation
Approximately two years of professional RN experience preferred
Previous Home Health or Hospice experience is preferred, but experienced RNs with strong transferable clinical skills are encouraged to apply. Experience with wound care, ostomies, IVs, infusion therapy and independent patient assessment is especially valuable.
