Visit RN – Home Health & Hospice | 0.8 FTE Days
Bring your nursing expertise directly to patients while having the autonomy to manage your day and the support of an experienced Home Health & Hospice team.
As a Visit RN with ProHealth Care, you’ll provide one-on-one care in patients’ homes throughout Waukesha County and portions of surrounding counties. This is a defined local service area, you are not traveling across the state.

Schedule & Territory
- 0.8 FTE | 32 hours per week | Day shift
- Flexible schedule
- Every third weekend with a holiday rotation of 2 of 6 holidays.
- Your territory will primarily include Waukesha County and portions of surrounding counties. Because this is a field-based nursing role, you’ll travel between patient homes throughout your workday. RNs should be comfortable driving approximately 50–70 miles during a typical day. Eligible work-related mileage is reimbursed.
What you’ll do
You’ll provide skilled nursing care to both Home Health and Hospice patients in their homes, allowing you to spend focused, one-on-one time with patients and families.
Your day may include:
- Comprehensive nursing assessments
- Medication management and reconciliation
- Wound care and other skilled nursing treatments
- IV therapy
- Patient and family education
- Care coordination with physicians and the interdisciplinary team
- OASIS and other required clinical documentation
- Start of Care and follow-up visits
- Supporting patients with both Home Health and Hospice needs
You’ll manage your assigned visits, plan your route and travel between patient homes throughout the day. Your ProHealth-provided laptop, cell phone and mobile hotspot help you document and stay connected to your team while working in the field.
What you’ll need
- Current Wisconsin RN license and nursing education/experience required by the Visit RN job profile.
- Because you’ll be providing care in patients’ homes, you’ll also need a valid driver’s license, current vehicle insurance and reliable transportation.
- Home Health, Hospice, OASIS, wound care and IV experience are valuable, but we also welcome experienced RNs with strong transferable clinical skills who are interested in learning Home Health & Hospice nursing.
You should also be comfortable working independently and entering a variety of home environments, which may include pets, smoke, allergens or other environmental factors.

What You Will Do:
As a Visit RN, you’ll work one-on-one with patients in their homes to provide skilled nursing care, coordinate services, and contribute to the patient’s overall care plan. You’ll collaborate with a dedicated team of nurses, social workers, physicians, caregivers, and community agencies to ensure a holistic, coordinated, and compassionate care experience.
- Deliver individualized, evidence-based nursing care in the home setting
- Assess patient status and communicate any changes in condition to the care team
- Document care in EPIC, our integrated electronic health record system
- Collaborate on discharge planning, care coordination, and resource utilization
- Serve as a clinical liaison with physicians and other members of the care team
- Educate patients and families on self-care, medications, and condition management
What You’ll Need:
- Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred
- Current WI RN license or eligibility through the eNLC compact
- Valid WI driver’s license and acceptable driving record
- BLS certification (or ability to obtain upon hire)
- Previous experience in home health or hospice is preferred
- Familiarity with EPIC and experience in case management or discharge planning a plus
- Strong interpersonal, communication, and critical thinking skills
