RN Case Manager – Home Palliative Care – 0.9 FTE Days

Palliative care nursing gives you something that can be hard to find in traditional bedside nursing: time to understand the whole patient, build relationships with patients and families, and help people manage complex health needs in the comfort of home.

As a Home Palliative Care RN Case Manager with ProHealth Care, you’ll combine hands-on nursing with care coordination, education and interdisciplinary collaboration. You’ll work independently in the field while remaining connected to physicians, social workers, nurses, caregivers and other members of the care team. Rather than caring for several patients at once, home-based care allows you to focus on one patient at a time.

Schedule

0.9 FTE – benefits eligible
Monday–Friday, 8:00 a.m.–4:30 p.m.
No regular weekends
Two holidays per year – one major and one minor
Backup on-call rotation approximately four times per month
Occasional on-call coverage for night RN PTO

What you’ll do

• Assess patients in their homes and develop individualized plans of care.
• Provide hands-on nursing care and continually evaluate patients’ progress and changing needs.
• Educate patients and caregivers about medications, symptoms, treatments and the plan of care.
• Coordinate services, referrals, discharge planning and community resources.
• Collaborate with physicians, social workers, nurses, caregivers and other health care professionals.
• Document assessments, interventions and patient responses in the medical record.
• Help patients and families navigate transitions across the health care continuum.
• Practice independently in the home while communicating closely with the interdisciplinary care team.

What you’ll need

• Associate Degree in Nursing required; Bachelor’s Degree in Nursing preferred.
• Current Wisconsin or eNLC compact-state RN license.
• Valid Wisconsin driver’s license with an acceptable driving record.
• Current BLS certification or ability to obtain upon hire.
• Clinical nursing experience preferred.
• Epic experience preferred.
• Strong assessment, communication, teaching and critical-thinking skills.
• Comfort working independently in patients’ homes.

$10,000 Sign-On Bonus - A $10,000 sign-on bonus with a two-year commitment is available for eligible candidates.
