Schedule Details: Position is for pool/as needed, minimum of 6 shifts per 4 week period with ability to pick up additional shifts according to department need. Shift time is 8:00am-4:30pm. Included in weekend and holiday rotation. This is an inpatient dietitian role and applicant must be able to travel between Waukesha, Oconomowoc and Mukwonago hospitals.
What You Will Do:
- Complete nutrition assessments and provide individualized medical nutrition therapy for hospitalized patients.
- Develop, implement, and adjust nutrition care plans based on each patient's clinical condition and nutritional needs.
- Collaborate with physicians, nurses, and other members of the healthcare team to support patient care.
- Educate patients and their families on nutrition-related conditions, treatment plans, and dietary recommendations.
- Monitor patient progress, evaluate outcomes, and document assessments and interventions in the medical record.
- Maintain current knowledge of evidence-based nutrition practices and standards of care.
- Serve as a preceptor for dietetic interns as assigned.
- Provide coverage and complete work responsibilities at ProHealth Care inpatient locations as needed.
